A social worker, an administrator, and an architect walk into a bar….so hopefully, we grabbed your attention! Collaboration is key to innovative change in environments for aging. You are influencers, identifiers of disparate parts, and the coalescers of connections among those parts leading toward a better future for living. We will share our journey supporting intergenerational care models and community to identify, verify, and provide components for successful aging. A universal, inclusive, and equitable approach to coordinated care supports multi-family housing projects and those aging at-home in community. Join us for an interdisciplinary conversation that focuses on thriving!


Through the looking glass: an interdisciplinary approach to person-centered care

The context of the discussion is the creation of an alternative intergenerational and inclusive community model called Live Together®. As part of the initiative, an Institute has been developed that includes knowledgeable advisors and the support of interdisciplinary models to maximize the living experience for residents of all ages. A panel of experts will provide their perspectives through the lens of their discipline and person-centered care as it positively impacts the delivery of care, services, research, and design of environments for aging within an intergenerational setting. The disciplines include nursing, social work, occupational therapy, recreational therapy, environmental services, data measurement, and design of supportive environments. The goal is to bring together these disciplines to identify commonality, but primarily to create and support inclusive care models within a built environment that truly supports residents, staff, and families - embracing that change is a constant factor for continuous improvement. · Intergenerational multi-family housing,
· Defining differences between dementia and Alzheimer’s,
· The importance of keeping the environment clean and safe,
· Strategies for providing Person Centered Care

I personally found the Strategies noted above very helpful for day-to-day communication with peers as well as with children and grandchildren. It is so logical that the environment (to include lighting, temperature and even clutter) effects some people more than others. But we often forget to be mindful of the impacts and the other Strategies discussed.
